Helen M. Forma of Cornwallville, NY passed away on Monday September 8, 2014 after a brief illness. She was born on March 25th 1917 in Yonkers, NY to Anthony and Albina (Stawiarski) Piller. She will be remembered to her friends and family as a Reader's Digest "Unforgettable Character". She is predeceased by her husband William Forma, her parents, and her siblings John Piller, Joseph Piller, and Mary Piller.

Helen is survived by her sons Robert Forma, William (Marian) Forma, and Steven (Caryl) Forma; her grandchildren Mark, Peter, Lisa, Adam, and Baird; and her great-grandchildren Hailey, Emily, Olyvia, Brennan, William, Anya, Ava, and Hugh.

Calling hours will be held on Friday September 12th 2012 from 4pm to 8pm with a prayer service at 7:30pm at A.J. Cunningham Funeral Homes, Inc, 4898 State Route 81 Greenville, NY.
